A little preparation can make your first visit more useful. Bring the information you have; contact the office if you are unsure what is needed.
Your first-visit checklist
- Photo identification and any information requested when scheduling.
- A current list of medications, supplements, allergies, and your pharmacy.
- Relevant medical records and imaging reports, when available.
- A short summary of symptoms, previous treatment, and your main questions.
Before your appointment
Confirm the appointment location, clinician, arrival time, visit fee, accepted payment methods, insurance arrangements, and cancellation policy with the office. Do not assume another clinic’s policies apply.
Patient forms
Open the fillable pain-history form. Download a copy before entering information, save your completed copy, and bring it to your visit or use the clinic’s approved delivery process.
What happens at the first visit?
The clinician reviews your concerns, history, previous care, and goals. Additional records, testing, follow-up, or referral may be discussed. An appointment does not guarantee a particular medication or procedure.
Your next step
Prepare for your first visit or review appointment options. Recommendations depend on your individual evaluation.
